Adoption Application

This form is a .pdf document.  You can fill this form out on your computer and submit it via email. The instructions below are somewhat Windows specific, although should be fairly similar for MacIntoshes and Linux-based systems.

  1. Download the form to your PC by right-clicking on the link below, selecting 'Save as...', navigating to your desktop, and save the file.
  2. Now go to your desktop and double-click on the file 'CPRApplicationPro.pdf'.  The application may open in Adobe Reader, Foxit, or whatever PDF viewer application you have installed.
  3. Fill out the form as completely as possible.
  4. Click 'File' in the toolbar at the top, select 'Save As', then 'PDF...'. Enter your name, with NO spaces, e.g. 'JohnDoe.pdf'.  BEFORE closing the PDF application, we suggest opening the file you've saved and verifying the information appears in it. You may also wish to print a copy and keep that until the adoption application has been processed.
  5. Open your email program (or web browser if you're using GMail, Hotmail, Yahoo!, etc), compose an email to 'adopt@caninepetrescue.com', put 'Adoption application for (dog's name)' , attach the saved PDF file ('JohnDoe.pdf'), and send it!

Note that there is a SUBMIT button at the very end of the PDF file that will also submit the application via your email program, but that does NOT work with GMail, Hotmail, Yahoo!, etc.  We recommend using the above procedure to ensure your application is properly submitted.
